In accordance with Texas law (House Bill 4224 and Texas Health and Safety Code ยง 181.105), this practice provides the following information for consumers:

Requesting Health Care Records To request a copy of your health care records from CG Counseling, please follow these instructions:

Submit a written request via email to administration@cgcounselingcenter.com.

  • Note: We will respond to your request within the timeframe required by state law.
